You need a referral first. Ring them, or ask a support worker, health visitor or Citizens Advice to refer you.
Their website says: Access to the service is through obtaining a Referral form (see below) from one of over 50 support organisations and agencies working in our community e.g. Mental Health Teams, CAB, Schools, Social Prescribers, Housing Providers, Local Counsellors and Local Authority Staff.
